COCC is pleased to
offer a comprehensive and competitive
benefit program to all eligible
employees who work 30 or more hours per
week. New employees are eligible for
benefits on the first of the month
following 30 days from your date of
hire.
Employees are eligible for the
following benefits:
Medical
COCC employees have the option of
choosing one of the following medical
plans:
HMO Hybrid – COCC currently
contributes 75% toward the employee and
family cost of this plan. Office visits
to your PCP require a $25 co-pay and
office visits to a specialist require a
$30 co-pay. Services must be performed
by a select group of contracted doctors.
Referrals are not required.
PPO Plan –This plan offers
in-network and out-of-network coverage.

co-pay and office visits to a specialist
require a $25 co-pay. Referrals are not
required.
HSA/High Deductible Plan – The
plan has a $1,500 Single/$3,000 family
deductible. After the deductible is met,
in-network services are covered at 100%,
Out-of-network services will be
reimbursed at a co-insurance rate.
Preventive services are covered at 100%
and are not part of the deductible. COCC
will also contribute 25% of the
deductible into an employee’s HSA
account spread out of the 27 pay periods
in 2009. Employees may choose to
contribute additional dollars into the
HSA account.
Voluntary Vision Plan – COCC employees
have the option to purchase vision
coverage through Anthem BCBS. The plan
provides yearly coverage for frames or
contact lenses up to a $120 per plan
year.
Dental
COCC offers two dental plans:
Dental Base Plan – COCC
contributes 75% towards the employee and
family cost. Members will have an annual
maximum of $1,000 per member.
Dental Buy Up Plan – COCC will
match the employer contribution of the
Dental Base Plan (75%) towards the cost
of the Dental Buy Up Plan. Members will
have an annual maximum of $2,000 per
member.
Life
and Accident Benefits
COCC provides coverage for term life
insurance and accidental death and
dismemberment to employees in an amount
equal to two times their salary at no
cost to the employees. Employees have
the opportunity to purchase additional
life insurance.
Short
Term Disability (STD)
If you become disabled and unable to
work, COCC provides Short Term
Disability benefits at a percentage of
your base salary for up to 13 weeks at
no cost to the employee.
Long
Term Disability (LTD)
If you exceed your Short Term Disability
coverage you will then be eligible for
Long Term Disability benefits at a
percentage of your base salary at no
cost to the employee.
Business Travel Accident Insurance
COCC offers Business Travel Accident
Insurance effective your date of hire if
you work 20 hours or more per week.
Flexible Spending Accounts (FSA)
COCC employees may participate in
Medical FSA or in a Dependent Care FSA.
Flexible Spending Accounts allow
employees to make pretax deductions from
their pay, which can be used to pay for
medical or dependent care expenses.
College Savings Plan
This program also known as a 529 College
Savings Plan can help you save towards
higher education for your children,
grandchildren nieces or nephews.
Contributions may be payroll deducted
and transferred to Connecticut Higher
Education Trust. (CHET)
401k
Plan
All employees who have attained age 20 ½
are eligible to participate in the 401k
Plan on the first of the month following
30 days from your date of hire. COCC
currently provides a special matching
contribution on employee salary
deferrals which are 100% vested.
Profit
Sharing Plan
COCC may make a discretionary
contribution to the Plan based upon the
profitability of the company provided
that the participant has worked one full
year and is employed on December 31st of
the Plan Year. A vesting schedule will
apply.
Paid
Time Off Program (PTO)
All employees who work at least 20 hours
per week become eligible for Paid Time
Off (PTO). PTO may be used for any
reason, such as vacation day, family
illness days, or individual sick time.
PTO allotment is determined by your
years of service. In addition employees
will receive a Paid Day Off For
Birthday. Company holidays are in
addition to PTO.
Tuition Assistance Program
COCC encourages professional development
in the workplace, therefore regular
fulltime employees and regular part time
employees who work 30 or more hours per
week are eligible to apply for tuition
assistance after successful completion
of six months of service. Reimbursement
is based on a percentage depending on
the enrolled plan.
Wellness and Fitness Program
Wellness programs are designed to help
you enhance your health and work
performance. COCC offers programs such
as wellness seminars; flu shot clinics
and discounted fitness memberships as
well as a Fitness Reimbursement Program.
Employee Assistance Program (EAP)
Employee assistance program provides
employees and their eligible family
members with wide-ranging information as
well as referrals to resources, health
professionals, counseling, etc.
Employees who are eligible for benefits
will have access to this service at no
cost to the employee.
Travel
Assistance Program
Whether your travel is for business or
pleasure, COCC offers a worldwide travel
assistance program to help you when an
unexpected emergency occurs. Employees
who are eligible for benefits will have
access to this service at no cost to the
employee.
